What credentials do I need to upload?

To start picking up shifts with Patriot, we need to ensure you have all your credentials to work in a facility! As well as credentials we have a series of documents to sign for you to be able to receive pay.

The documents include an I9, W4, Employee Handbook, and a temporary employee contract. The credentials necessary to work include:

• Active Working Licenses (CNA, CMA, LVN, RN)

• TB Test

• COVID Vaccine / COVID Exemption form (We can provide)

• Driver’s License

• Social Security Card

• CPR / BLS Certification
